1. What is the goal/mission statement of your practice?
2. What is your specialty?
3. What is your educational background?
4. How long have you been practicing medicine?
5. What alternative treatment services does your practice provide, if any? For example, acupuncture, massage, etc.
6. What should a patient bring to their initial appointment?
7. What can a patient expect at their initial appointment?
8. What forms of payment do you accept?  What is your fee structure?
9. Do you accept any forms of insurance?
10. Do you offer any type of hardship, veteran or disability discount?
